Putting You and Your Family at the Centre of the Primary Healthcare Services

We are strengthening primary care by bringing together the North Shore community, its family doctors, and Vancouver Coastal Health to collectively drive health and wellbeing. We are physicians, clinicians, and program administrators in the North Shore Division of Family Practice working in collaboration and partnership with the Vancouver Coastal Health Authority, Squamish and Tsleil-Waututh First Nations, and many of the community agencies that serve people throughout North Vancouver, West Vancouver, and Bowen Island; from seniors and families to newcomers and the most vulnerable populations.

We are creating multiple linkages from family practices to community agencies so that patients’ experience of healthcare teams not only includes doctors but also their community agency providers. We are re-imagining health care and integrating social supports to better care for you and your family. The North Shore PCN is based on a co-developed, sustainable vision for the future of healthcare to enable access to quality care that effectively meets the needs of North Shore residents and patients.
