The North Shore PCN is compiling a list of reliable resources and supports on health and wellbeing for you and your family. Many of these resources are free or low-cost. Contact the organization or check their website to determine service eligibility.

Always remember that online health information and resources can be convenient to use. But remember, it is important to ensure the resources you consult are reliable and up-to-date.

North Shore PCN Community Directory

Pathways: Community Service Directory – North Shore Pathways is an online resource that provides people living on the North Shore access to hundreds of patient resources, with a searchable community service directory.

Foundry North Shore
Foundry offers in-person and virtual healthcare services for youth 12-24 (and their families or caregivers). It provides access to mental health and substance use support, primary care, peer support, and social services—all without needing a doctor’s referral. Drop by today!

HealthConnection Clinic (HCC) 
HCC is an adult service (19+), providing primary care services to individuals who do not have a regular family Family Physician or Nurse Practitioner, and who face complex medical, mental health, addiction, and/or socioeconomic needs (e.g., housing, income, access to food). No scheduled appointment or formal referral is necessary to be seen at the clinic; however, scheduled appointments are available.

Mental Health Resources

Mental health concerns us all. Mother, father, neighbour, friend – one in five Canadians will experience a mental health problem or illness every year. Many people either don’t seek or can’t get the services and support they need to recover and live a meaningful life.
