North Shore Family Physicians are able to refer patients for social prescription reasons to our partners, North Shore Community Resources. Social prescribing programs allow family physicians to refer patients to local agencies that help address non-medical needs through activities like exercise classes and social events. These programs provide follow-up support to monitor progress and enhance overall well-being, addressing both physical health and emotional wellness.
Recently, one of our family physicians referred an older adult for Social Prescription. They prescribed assistance with finances, social support and navigation of government financial support. North Shore Community Resources found that this person was experiencing significant financial hardship with a shortfall per month, leading to missed meals due to lack of food in the house.
The navigator assisted the person to apply for Service Canada funding to ensure that all government benefits and monthly pension were signed up for. This led to an application for low income housing in close proximity to their children. They were missing clinic appointments, and not getting out of their house, leading to social isolation.
The navigator helped them to apply for a reduced cost bus pass, and called the Salvation Army to complete an application and deliver a bag of food on the same day. They are now able to bus to appointments and social activities.
