What is a PCN Clinical Pharmacist?

PCN pharmacists provide comprehensive medication management for adults with complex health conditions. Their goal is to prevent and resolve medication-related issues while ensuring you receive the safest and most effective treatment. They also educate patients about their medications and help them follow their prescribed treatment plans.

Working closely with you and your doctor, a PCN pharmacist can:

  • Manage medications for conditions such as diabetes, chronic pain, high blood pressure, high cholesterol, asthma, and COPD
  • Help you create a personalized action plan to achieve your health goals
  • Provide education on natural medicines and supplements
  • Offer guidance and resources to help you quit smoking
