What is a Primary Care Network?
Primary Care Networks are transforming health care in BC from a traditional, siloed system to one that is robust and integrated, aimed at effectively meeting your needs. Many people in BC are unable to get a family doctor or timely access to the full range of timely care.
The North Shore Primary Care Network (PCN) is a local network of interconnected family healthcare practices, community health centres, and walk-in clinics that we are actively building in our community. The North Shore PCN community includes North and West Vancouver, as well as Bowen Island. As a patient who lives and works in these communities you and your family will receive expanded, culturally safe, comprehensive care through the PCN, and increased access to a regular family physician or nurse practitioner, known as primary care providers. Doctors and other healthcare professionals work collaboratively to provide integrated care for all your primary healthcare needs.
Primary Care Networks work better for patients, so people can get medical care when and where they need it. It’s also great for providers as they are able to care for their patients while being supported by an interdisciplinary team of providers within a community network.

A New Team-Based Model of Family Medicine
The North Shore PCN takes a team-based approach that places patients of North Shore Primary Care Providers within a circle of care. At the centre we have primary care, including family physicians, allied health, and nursing, as well as additional facilities that house primary care on the North Shore, such as Urgent and Primary Care Centres.
Patient Medical Homes (PMHs) serve as the foundation of the primary care system. A patient medical home is a family practice that provides life-long patient care. Primary care providers are healthcare professionals, such as a family physician (FP) or a nurse practitioner (NP), who help you manage your health. A long-term relationship with a primary care provider keeps you healthier; they can treat you when you are sick or help you access more advanced care when you need it.
