North Shore PCN Partners

The North Shore PCN is a partnership between the North Shore Division of Family Practice representing North Shore family physicians, the Vancouver Coastal Health Authority, the Squamish (Sḵwx̱wú7mesh), and Tsleil-Waututh (səlilwətaɬ) First Nations, as well as local community organizations. All partners are working together to redesign our primary and community care system so that it works better for both patients and healthcare providers.
