North Shore Primary Care registered nurses (RNs) are important team members in providing collaborative, interdisciplinary team-based care. RNs work side-by-side with the physician and other health care professionals as partners and are crucial to the team’s achievement of better screening and health promotion, early detection, chronic disease management and enhanced access. The RN permits your family doctor to provide quicker access to comprehensive support for patients, especially those who are frail, elderly, and/or have complex and chronic conditions. This support enables family doctors to focus on things like building relationships with you as a person and spending more time on difficult diagnostic dilemmas.
Some of the responsibilities of an RN in Practice include:
- Performs comprehensive and focused assessments
- Administers Vaccinations
- Prioritizes referrals and screening of patient information
- Conducts Complex care and Chronic Disease Management visits (CDM)
- Wound care and follow-up visits

One North Shore family who was looking for strategies to manage care at home and to register their elderly relative for Long Term Care. They met with Elisabeth Ash, an RN in Practice at a local NS clinic with a background in Gerontology. Elisabeth not only helped them to organize respite care, a home safety assessment, Home Support, and helped prepare the “Patient Medical Pre-Admission to Care Facility” form, but she also conducted a cognitive screening test to assess how safe the person is at home. The family member stated “Elisabeth, this has made such a difference! My Mum is doing so much better, thanks to your suggestions and connections to supports.”
