Who we are
The North Shore Primary Care Network (NSPCN) is enabled by a partnership between the North Shore Division of Family Practice , Tsleil-Waututh and Squamish First Nations, Vancouver Coastal Health, BC Ministry of Health and community service providers. The NSPCN is based on a co-developed, sustainable vision for the future of health care to enable access to quality care that effectively meets the needs of North Shore residents and patients.


What is the North Shore Division of Family Practice?
The NSPCN is delivered by the North Shore Division of Family Practice, which is a non-profit society whose members are family physicians on the North Shore.
The North Shore Division supports and advocates for family physicians, works to expand access to primary care, and is committed to improving health outcomes for patients. It’s core mission is to ensure physicians have the tools and resources they need to care for their patients effectively.
The North Shore Division also plays a key role in driving system change, ensuring that family physicians remain central to the evolving health care landscape in our community.
Our PCN vision
The vision of the North Shore Primary Care Network is to lead the delivery of primary care services to the North Shore through collaboration, innovation, and team-based care to ensure healthy patients and satisfied health providers.
Our PCN mission
To build and sustain a community of patient-centred medical homes that integrate with the overall healthcare system to provide North Shore patients with the best care.
What we do
The NSPCN increases the capacity to provide primary care. We link patients to other parts of the North Shore healthcare system, including the Health Authority’s specialized community services for vulnerable people, such as the frail elderly and those who have mental health issues, or substance use concerns.
Research shows that people who have a family doctor are healthier, live longer, and are less likely to become hospitalized. We are here to help build on the strength of doctor-patient relationships and work alongside the more than 287 North Shore family doctors to provide support for continuous health care. Doctors within the Primary Care Network do so with their patients in mind. We develop innovative local health programs and services that enable physicians and healthcare teams to stay connected to their patients and provide better care.
